OK, I think I know what has been causing the problems. It's some of the new mobs in Field of Silence. I noticed sometimes they will respawn on top of those stone looking structures and then the minions will respawn repeatedly.
I'm going to remove all of them and reboot. Hopefully that is what has been the problem.
